Inclusion criteria

Inclusion criteria: Stroke patients in phase C/D

Exclusion criteria

Exclusion criteria: Patients with severe motor and cognitive impairments. Patients who cannot return to their previous home environment after completing the medical rehabilitation, due to serious impairments (for example, short-term care or a nursing home).

Measure	Time frame
The goal of this study is to use a modified questionnaire to collect experiences and desires of stroke patients and relatives. Data collection is carried out for a period of 5 months. In addition, the clinical experts are interviewed with a short online survey. Data collection is carried out for a period of 9 weeks.	—
