Inclusion criteria
Inclusion criteria: Stroke patients in phase C/D
Exclusion criteria
Exclusion criteria: Patients with severe motor and cognitive impairments. Patients who cannot return to their previous home environment after completing the medical rehabilitation, due to serious impairments (for example, short-term care or a nursing home).
