Group 1: At the end of the rehabilitation we define with all participants of the study aims for the following twelve months. Furthermore all participants get a course of instruction in the internet. T

Inclusion criteria
Inclusion criteria: Patients who completed a stationary rehabilitation in one of the four medical centers • with following diagnose(s): - main diagnosis adiposity (bmi=30) or - main diagnosis diabetes type 2 and secondary diagnosis adiposity or - orthopedic main diagnosis and secondary diagnosis adiposity • with technical and cognitive requirements to use the internet • who sign the declaration of agreement to participate in the study • who have sufficient knowledge in the German language (in particular in writing)
Exclusion criteria
Exclusion criteria: • status after bariatric surgery • severe psychiatric disorder (e.g. at risk of suicide, severe eating disorder) • severe somatic disease (cancer, status after LTX), that has a greater impact on the prediction of the patient than the metabolism disease
