Inclusion criteria
Inclusion criteria: Age 18-65 years (not retired); Diagnosis: mental and behavioural disorders (ICD F00-F99); Completed inpatient rehabilitation treatment and approval/referral for outpatient follow-up care
Exclusion criteria
Exclusion criteria: Insufficient knowledge of the German language; Acute psychiatric, dementia or acute disease with urgent need for medical treatment that postpones or counteracts rehabilitation; Patients whose onset of ambulant after-care is already prior to the start of the survey are not included in the study

| The primary target criterion is the motivation for a regular termination of ambulant after-care is measured at the beginning (T0) and at the end (T1) of after-care using standardized questionnaires. Participation motivation is assessed using a questionnaire (start of follow-up care (T0) and six months after onset of after-care (T1)). In the intervention group the influence of a therapy book on the increase in motivation for the regular termination of ambulant aftercare is examined. The questionnaire T0 contains the following scales: The scales of emotional support (EU, 16 items) and social integration (SI, 13 items) from the questionnaire on social assistance (F-SozU), the symptom checklist (SCL-90, 90 items) as well as 7 sociodemographic items. In addition, 13 items of a self-developed scale question the expectation of ambulant after-care and rehabilitation-related parameters. The questionnaire T1 contains the following scales: The scales of emotional support (EU, 16 items) and social integration (SI, 13 items) from the questionnaire on social assistance (F-SozU), the symptom checklist (SCL-90, 90 items) as well as 7 sociodemographic items. A self-developed scale (27 items) records the experience with ambulant after-care. | — |

| Secondary endpoints are the perceived social and emotional support, the self-assessment of the state of health by the patient himself, as well as the expectations and the perceived effect of the ambulant after-care.
