Interventions

Group 1: Multiple measurements with Magnetic Resonance Scanner within up to 24 hours after the stroke
serial measurement of the ASL/DWI (arterial spin labeling/diffusion weighted imaging) MRI (magnetic resonance imaging) Mismatch. Study with 3 steps, 25 patients per step.

18 Years to 85 Years

Inclusion criteria

Inclusion criteria: 1. 18-85 years 2. Acute ischemic stroke of the anterior circulation within 4.5 (Step II) or 9 hours (Steps I and III) of symptom onset. 3. Clinically stable state 4. NIHSS =2 5. Signed informed consent of the patient or legal guardian

Exclusion criteria

Exclusion criteria: 1. MRI contraindication 2. pregnancy (betaHCG urin/serum) 3. endovascular or other operative procedures planned within next 24hrs. 4. life expectancy <3months 5. Diagnosed intracerebral bleeding before study inclusion 6. no informed consent obtainable, no legal guardian 7. examination with MR-contrast agent (gadolinium derivate) less than 24hrs before study inclusion.

Design outcomes

Primary

MeasureTime frame
Serial/longitudinal change in ASL/DWI Mismatchvolume. Step I: 5 consecutive MRI scans within 1 hour, as soon as possible after symptom onset. 1 Follow-Up MRI after 24h (+/- 3h). Step II: 1. MRI scan at baseline as soon as possible after symptom onset. MRI scans at 1.5, 3, 4.5, 6, 7.5, 9hrs fter symptom onset. If inclusion after 3hrs, no measurment at 1.5 and 3 hrs. 1 Follow-Up MRI after 24h (+/- 3h). Step III: 1. MRI scan at baseline as soon as possible after symptom onset. MRI scans at 9, 12, 16, 20, 24hrs after symptom onset.
