Inclusion criteria

Inclusion criteria: - membership to the German Pension Insurance - aged between 18 and 65 - in-patients with the following diagnoses: 1. surgically treated bone fracture 2. hip and knee endoprosthesis 3. malignant diseases in the visceral surgery 4. heart diseases without operative intervention 5. heart diseases with operative intervention 6. stroke-patients who can discharged for rehabilitation. Furthermore, physicians and employees of the hospital social services will be included for a separate survey.

Exclusion criteria: Patients of early rehabilitation
