Inclusion criteria

Inclusion criteria: Patients of the medical practices participating or patients of the emergency department at Fürth hospital.

Exclusion criteria

Exclusion criteria: Patients under 18 years. Patients referred from practices participating in the inter-sectoral exchange of medication data via the medication plan (eMediPlan) who do not give consent for the exchange of medication data via the medication plan (eMP).
