Inclusion criteria

Inclusion criteria: For the HC invited insured chonically ill people with hospital stay in the last 4-6 weeks (COPD, Asthma, Coronary Heart Disease, Diabetes, Hypertension, Cardiac Insufficiency).

Exclusion criteria

Exclusion criteria: Insured people staying in hospital.

Design outcomes

Primary

MeasureTime frame
The primary outcome is the time till rehospitalisation. It will be assessed in days through the routine data of the health insurance.

Secondary

MeasureTime frame
Rehospitalisation rate assessed throuh routine data; number of days in hospital Health related quality of life (SF-36), 12 months and 36 months after beginning of coaching Reduction of risk factors and risky health behaviors (exercise, nutrition, smoking, alcohol) assessed throuh questionnaire, Health Locus of Control (MHLC) 12 months and 36 months after beginning of coaching
