Group 1: Allocation on waiting time, without prospective matching for HLA antigens (one kidney
current practice in ESP) Group 2: Allocation based on HLA-DR compatibility (defined by zero HLA-DR mismatches) followed by waiting time (the other kidney, ESDP)

Inclusion criteria

Inclusion criteria: 1. men or women aged at least 65 years, placed on eurotransplant kidney waiting list with an active urgency; 2. recipients of a first kidney; 3. highest (historic or current) panel reactive antibody (PRA) titers of 5%; 4. negative cross-matches and (if applicable) unacceptable antigens are excluded.

Exclusion criteria

Exclusion criteria: 1. active systemic infection; 2. HIV (antigen or antibody), hepatitis-B (HBsAg), or hepatitis-C (antibody) positivity; 3. history of malignancy within 5 years of enrolment (with the exception of adequately treated non-melanoma skin cancer); 4. known to be poorly compliant with clinic visits or prescribed medication; 5. medical history that might limit the individual's ability to take the defined immunosuppressive agents.

Validation of donor kidney allocation due to assessment of acute rejection 6 month post-transplantation.
