Inclusion criteria
Inclusion criteria: Patients: Fulfulling one of the diagnoses; provider: provider of a rehabilitation clinic or family physician
Exclusion criteria
Exclusion criteria: Language impairment, cognitive and/or physical impairment (patient/provider is not able to fill out questionnaire or to be interviewed)
