Inclusion criteria

Inclusion criteria: Patient is enrolled in the primary care scheme provided by the AOK Baden-Württemberg, Germany; Patient is counseled by his/ her general practitioner/ family physician using arriba.

Exclusion criteria

Exclusion criteria: no exclusion criteria

Design outcomes

Primary

MeasureTime frame
During follow-up data on primary outcome (composite acute coronary syndrome and/ or stroke) will be derived from administrative data bases of the AOK Baden-Württemberg.

Secondary

MeasureTime frame
During follow-up data on secondary outcomes (overall death, hospitalisation for cardiovascular disease, any hospitalisation, revascularization procedures,cardiology consultations) will be derived from administrative data bases of the AOK Baden-Württemberg.
