Inclusion criteria
Inclusion criteria: Inclusion criteria for pediatric patients: 1. Aged >=12 and =1 year; 3. Provide informed consent for voluntary participation.
Exclusion criteria
Exclusion criteria: Exclusion criteria for child patients: 1. Complicated with severe cognitive or mental disorders, unable to complete the main questionnaire modules with no available assistance; 2. Critically ill or in the acute episode of disease; 3. Participation in similar transitionalcare surveys within the past 3 months; 4. Fully transferred to the adult healthcare system and cannot be reached via the original pediatric team. Exclusion criteria for parents: 1. Not the primary caregiver of the child patient; 2. Lacking readingwriting and comprehension skills with no available assistance for questionnaire completion. Exclusion criteria for medical staff: 1. Interns or visiting physicians with less than 1 year of working experience; 2. Have never treated adolescent patients with chronic urinary diseases requiring transition to adult departments.
