Inclusion criteria

Inclusion criteria: 1.First diagnosed with ACS, meeting the diagnostic criteria of the Emergency Rapid Diagnosis and Treatment Guidelines for Acute Coronary Syndrome (2019), and undergoing PCI; 2.Patients on polypharmacy, defined as using at least 5 long-term prescription medications after discharge; 3.Patients with the ability to communicate and who provided informed consent for this study;

Exclusion criteria

Exclusion criteria: 1.Patients with impaired consciousness or mental illness; 2.Patients with severe cardiac, hepatic, pulmonary, renal, or cerebral dysfunction; 3.Patients transferred to another hospital for continued treatment after discharge; 4.Patients whose neither themselves nor their caregivers are capable of normal verbal communication;
