Inclusion criteria

Inclusion criteria: 1. Diagnosed with at least one chronic disease; 2. Taking 5 or more medications; 3. Permanent community residents willing to comply with follow-up.

Exclusion criteria

Exclusion criteria: 1. Severe cognitive impairment or psychiatric illness that precludes cooperation; 2. Life expectancy of less than 12 months; 3. Recent participation in other drug trials.
