18 Years to 80 Years
Inclusion criteria
Inclusion criteria: 1.Age >=18 years; 2.Glycated hemoglobin (HbA1c) >=7.0%; 3.Having lived locally for at least 6 months and having no plans to relocate within the next year; 4.Voluntary participation in the study and provision of written informed consent.
Exclusion criteria
Exclusion criteria: 1.Age >=80 years; 2.A history of myocardial infarction, stroke, heart failure, or severe arrhythmia, or previous percutaneous coronary intervention (PCI) or coronary artery bypass grafting (CABG); 3.Severe diseases such as cancer or severe hepatic or renal dysfunction; 4.Pregnancy or breastfeeding, or planning to become pregnant within 12 months; 5.Cognitive impairment, communication difficulties, or limited mobility; 6.Considered unsuitable for participation in the study by an expert or physician;
