Inclusion criteria
Inclusion criteria: Quantitative research: Inclusion Criteria for Clusters 1. The community can recruit more than 15 diabetes patients. 2. The community has not implemented similar projects. 3. The community agrees to participate in the project for at least 3 months. 4. The community agrees to assign at least one full-time or part-time project leader to implement the management and intervention plan. Inclusion Criteria for Individuals 1. Diagnosed with diabetes by a secondary or higher-level hospital. 2. Aged 18 years or above. 3. Diagnosed with diabetic peripheral neuropathic pain (DPNP) by I-DN4 scale (score =3) or at least one abnormality in the five physical examinations. 4. Owns a smartphone and can independently operate it to send text messages, make and receive calls, use WeChat chat, etc. 5. Voluntarily participates in the 3-month DPNP intervention and management program. Qualitative Study: 1. Patients Convenience sampling was used to select patients from the control group and intervention group for interviews. In accordance with the principle of data saturation, interviews were stopped when no new information emerged. The sample size of this study was 10–15 participants per group. 2. Healthcare Workers 1. Physicians, nurses or other relevant medical staff and managers with certain experience in the treatment or management of DPNP patients. 2. Understand and have used the WeChat Work platform, and can discuss its practical application and impact in depth. 3. Understand and have participated in the application of continuous glucose monitoring (CGM) devices in DPNP patient management, and can share views on data interpretation and impact on pain management. 4. Understand self-management group activities and can put forward professional suggestions for activity improvement. 5. Have strong desire for expression and communication skills, and can clearly express views, effectiveness evaluation and application experience on the pain management model, WeChat Work platform, CGM devices and self-management group activities. 6. Voluntarily participate in the study and understand and agree to the purpose and procedures of the qualitative interview.

Severe intellectual disability, severe cognitive impairment, or other serious mental health problems. 2. Language or communication barriers. 3. Ineligible for use of continuous glucose monitoring (CGM) devices. 4. Planned participation in other intervention or management programs during the study period. 5. Refusal to participate in this study. 6. Currently participating in other clinical studies. Qualitative Study Exclusion Criteria 1. Patients Those unwilling to participate in this study. 2. Healthcare Workers Those unwilling to participate in this study.
