Interventions

Gold Standard:Right heart catheterization (RHC) combined with comprehensive clinical assessment was used as the reference standard.
Index test:AI-based chest CT analysis system integrating pulmonary vascular, lung parenchymal, and airway features for COPD-associated pulmonary hypertension detection.

Inclusion criteria

Inclusion criteria: 1. Age >=18 years; 2. Diagnosis of chronic obstructive pulmonary disease (COPD) according to established diagnostic criteria; 3. Availability of chest CT examinations with image quality sufficient for analysis; 4. Availability of complete or relatively complete clinical data, including demographic information, pulmonary function tests, laboratory examinations, echocardiography, and/or right heart catheterization (RHC) data; 5. Patients with follow-up data will be preferentially included in the longitudinal imaging evolution analysis cohort; 6. The COPD-PH group must meet PH diagnostic criteria (mPAP >20 mmHg measured by right heart catheterization), and have a clinical diagnosis of COPD-associated PH; 7. The control group must have no history of chronic pulmonary or pulmonary vascular diseases, no significant structural abnormalities on chest CT, and available basic clinical information.

Exclusion criteria

Exclusion criteria: 1.Presence of other types of pulmonary hypertension (PH), including but not limited to connective tissue disease-associated PH, chronic thromboembolic pulmonary hypertension (CTEPH), and congenital heart disease-associated PH; 2.Coexisting severe interstitial lung disease, pulmonary malignancy, extensive lung resection, or other diseases that may significantly affect lung structural assessment; 3.Chest CT images with severe motion artifacts, inappropriate slice thickness, or missing data that preclude reliable image analysis; 4.Severe missing clinical information preventing extraction of key study variables; 5.Pregnant or breastfeeding patients (if applicable to the prospective component of the study); 6.Patients considered unsuitable for inclusion by the investigators.
