Inclusion criteria

Inclusion criteria: Part 1: 1. Patients aged 18 to 100 years old, no gender restriction 2. Patients presenting with two of the following conditions: hypertension, diabetes mellitus, obesity, hyperlipidemia, hyperuricemia, insomnia, carotid artery stenosis, thyroid diseases, previous stroke history, coronary heart disease, atrial fibrillation, and history of myocardial infarction. 3. Previous hospitalization for ischemic stroke Part 2: 1. Subjects aged 18 to 100 years old, with no restriction on gender. 2. Long-term residence in Fuzhou, Linyi, Minhang District of Shanghai and Putuo District of Shanghai 3. Have undergone physical examinations under routine management of the Comprehensive Demonstration Zones for Prevention and Control of Non-communicable Diseases (including cardiovascular diseases, stroke, diabetes mellitus, hypertension, chronic kidney disease, chronic respiratory diseases, etc.) designated by the Chinese Center for Disease Control and Prevention for more than 2 years. 4. High-risk stroke population who meet the criteria of the stroke prediction model for high-risk populations in eastern China described in Part 1. 5. Agree to participate in this study and sign the informed consent form

Exclusion criteria

Exclusion criteria: Part 1: 1. Unclear definite diagnosis of stroke, such as cerebral ischemia, vertigo, limb numbness, etc. 2. Incomplete documentation of stroke subtype, e.g., diagnosed as cerebrovascular disease, apoplexy, etc. Part 2: 1. Subjects with unstable residential address or unable to comply with regular physical examinations and grid-based management within the non-communicable disease comprehensive prevention and control demonstration zones. 2. Subjects already participating in other research projects focusing on pre-hospital prevention-related health education, emergency care pathways, or stroke recanalization therapy. 3. Subjects unable to complete study follow-up due to severe cognitive impairment, advanced malignant tumors, limited behavioral capacity, or other conditions.
