Inclusion criteria

Inclusion criteria: 1.Residents aged =35 years in Guangzhou; 2.Had traceable records in the resident health record system, Guangzhou chronic disease management system, or relevant healthcare information systems; 3.Provided written informed consent and agreed to participate in baseline assessments, intervention management, and long-term follow-up;

Exclusion criteria

Exclusion criteria: 1.Had mental disorders or cognitive impairment, including dementia, impaired comprehension, or other conditions affecting their ability to participate; 2.Had severe physical illnesses; 3.Were unable to complete the core baseline data collection or lacked essential identification information and contact details required for follow-up; 4.Had a life expectancy of less than 6 months; 5.For participants assigned to the wearable device–based precision management group, those who were unable to wear the device or complete wearable device data collection were excluded;
