Interventions

Intelligent Management Group:Patients will be managed by a specialized diabetes care team, with the implementation of an intelligent blood glucose management system and standardized service pathways.
Routine Management Group:Patients are managed using a conventional diagnosis and treatment model.

Inclusion criteria

Inclusion criteria: 1.The inclusion criteria for the research content in Part 1 are as follows:; 1.Permanent residents who have lived in the community for at least 6 months (verified by household registration, residence permit, or lease agreement); 2.Aged >=18 years; 3. Those who have provided informed consent and voluntarily agree to participate in this study. 2.The inclusion criteria for the research content in Part 2 are as follows:; 1.Confirmed diagnosis of type 2 diabetes mellitus; 2.Permanent residents who have lived in the community for at least 6 months; 3.Aged between 35 and 80 years; 4.Those who voluntarily agree to participate in this study and are able to complete follow-up.

Exclusion criteria

Exclusion criteria: 1.The exclusion criteria for the research content in Part 1 are as follows:; 1.Individuals with severe mobility impairments; 2.Those with severe mental disorders; 3.Other individuals unable to cooperate with the survey; 4.Pregnant or lactating women; 5.Those who have experienced acute infection, trauma, surgery, or other stressful conditions within the past 3 months. 2.The exclusion criteria for the research content in Part 2 are as follows:; 1.Individuals with high mobility, poor compliance, or inability to complete follow-up; 2.Patients with type 1 diabetes; 3.Individuals with severe mobility impairments; 4.Transaminase levels >3 times the upper limit of normal; 5.Individuals with severe renal insufficiency (eGFR < 30 mL/min/1.73 m^2); 6.Individuals with malignant tumors or severe mental disorders; 7.Individuals with other major diseases.
