Inclusion criteria

Inclusion criteria: 1. A confirmed diagnosis of type 2 diabetes mellitus with a disease duration of >= 5 years; 2. Persistent normoalbuminuria at baseline (UACR = 90 mL/min/1.73m² with documented evidence of an annual eGFR decline rate > 5 mL/min/1.73m², which should be validated by two eGFR measurements separated by a minimum of 12 months; 4. Presence of at least one DKD high-risk factor, including HbA1c > 7.5%, a history of hypertension, or any grade of diabetic retinopathy.

Exclusion criteria

Exclusion criteria: 1. Known or suspected non-diabetic renal diseases; 2. A diagnosis of type 1 diabetes mellitus; 3. Severe ocular diseases (other than diabetic retinopathy) that may substantially affect fundus imaging quality; 4. Failure to provide informed consent or complete follow-up.
