Inclusion criteria
Inclusion criteria: 1. Patients aged >=35 years with type 2 diabetes mellitus confirmed by hospital diagnosis; 2. Having resided locally for more than 1 year with no plan to relocate within the next 2 years; 3. Receiving regular health management services for type 2 diabetes from the community health service institution, capable of periodic follow-up visits, with self-care ability, intact cognitive function and normal verbal communication skills; 4. Voluntarily participating in this study after signing the informed consent form.
Exclusion criteria
Exclusion criteria: 1. Subjects with severe acute or chronic diseases or mental illnesses who are unable to cooperate with the research; 2. Patients with peripheral neuropathy induced by other illnesses (such as uremic neuropathy, alcoholic neuropathy, cerebrovascular accident, etc.); 3. Individuals incapable of participating in all activities of the project; 4. Patients with pre-existing diabetic foot disease.
