Inclusion criteria
Inclusion criteria: 1.Aged 65 years or older (elderly population). 2.Presence of comorbidity: combination of two or more diseases. 3.A clear history of chronic diseases, such as hypertension, diabetes, hyperlipidemia, chronic respiratory diseases (e.g., asthma, chronic obstructive pulmonary disease), cardiovascular diseases, etc. 4.Patients must have at least five years of experience in chronic disease management. 5.Medical records showing involvement of multiple clinical departments in diagnosis and treatment. 6.Regular electronic medical follow-up records, with the ability to provide medical data over an extended period.
Exclusion criteria
Exclusion criteria: 1.Patients with a history of severe surgery or interventional treatment. 2.Patients with a history of severe diseases: such as stroke, severe heart disease, severe renal insufficiency, liver disease, or other serious complications. 3.Poor chronic disease management: such as inability to take medication as prescribed, difficulty with insulin injection, etc. 4.Genetic factors: patients with a family history of hypertension, diabetes, or other polygenic disease-related factors.
