Inclusion criteria

Inclusion criteria: 1. Patients aged >=65 years scheduled to undergo elective radical surgery for gastrointestinal cancer; 2. Availability of a non-contrast abdominal CT scan acquired within 3 months before surgery that is suitable for analysis; 3. Able to cooperate with and complete postoperative delirium assessments;

Exclusion criteria

Exclusion criteria: 1. Preoperative cognitive or communication impairment: - Documented diagnosis of dementia (e.g., Alzheimer's disease, vascular dementia, etc.). - Severe hearing or visual impairment that precludes effective communication required for preoperative cognitive assessment or postoperative delirium evaluation. - Severe cognitive impairment confirmed by a brief preoperative cognitive screening (e.g., Mini-Cog), judged by the investigator as incompatible with compliance with study procedures. - Preoperative delirium confirmed by the Confusion Assessment Method (CAM) at the time of the preoperative visit. 2. Severe central nervous system comorbidities: - History of severe psychiatric disorders (e.g., schizophrenia, acute episode of major depressive disorder). - History of clinically confirmed stroke or severe traumatic brain injury within the recent 3 months. - Advanced Parkinson's disease or other neurological disorders that severely affect cognitive function. 3. Conditions affecting accurate body composition measurement: - Severe metal artifacts, motion artifacts, or extensive tumor infiltration on the CT image at the third lumbar vertebra (L3) level, rendering clear delineation of abdominal and back muscle contours unfeasible. - Known neuromuscular diseases that affect muscle mass or fat distribution (e.g., amyotrophic lateral sclerosis, myasthenia gravis, polymyositis, etc.). 4. Critical preoperative status: - Admission to the intensive care unit requiring life support for any reason before surgery. - American Society of Anesthesiologists (ASA) physical status classification V (moribund patient not expected to survive without the operation). - Emergency colorectal surgery. 5. Miscellaneous: The patient refuses to participate in this study or is unable to sign the informed consent form.
