Inclusion criteria

Inclusion criteria: 1. Elderly patients (aged >=60) enrolled in the family doctor studio program, who are in good mental state and have no communication barriers; 2. Require long-term medication (>=1 year); 3. Diagnosed with at least one type of chronic non-communicable disease, including stable-phase cardiovascular or cerebrovascular conditions, type 2 diabetes, etc., and with stable vital signs in other respects; 4. Concurrently taking >=3 types of medications (including prescription drugs, Chinese herbal medicines, over-the-counter drugs, and other dietary supplements).

Exclusion criteria

Exclusion criteria: 1. Patients with end-stage diseases or critical illnesses; 2. Those with hepatic or renal dysfunction, or other severe complications; 3. Those with severe cognitive or mental disorders.
