Inclusion criteria
Inclusion criteria: 1. Aged >= 18 years; 2. Confirmed by a secondary or higher-level hospital, the patient suffers from at least two chronic diseases (refer to the WHO or China's catalog of common chronic diseases for definitions) with stable conditions. 3. Currently taking at least two long-term medications; 4. The patient is fully conscious, possesses basic reading, writing, listening and speaking skills, and is able to understand the survey content. 5. Participate voluntarily and sign the informed consent form; 6. Master the basic operations of smartphones.
Exclusion criteria
Exclusion criteria: 1. Persons with severe cognitive impairment or mental illness who are unable to cooperate with the survey; 2. Patients in the acute phase of illness or in critical condition; 3. During the survey, respondents who are unable to finish the remaining questionnaires due to changes in their medical condition or other reasons.
