Inclusion criteria
Inclusion criteria: 1. Meets the 2023 WHO criteria for diabetes diagnosis (fasting blood glucose >=7.0 mmol/L or HbA1c >=6.5%); 2. Confirmed diabetic peripheral neuropathy (Toronto clinical score >=5 points); 3. Presence of neuropathic pain (DN4 scale >=4 points, VAS >=4 points, lasting >=3 months); 4. Abnormal nerve electrophysiology (sural nerve conduction velocity <40 m/s or amplitude <1 µV); 5. Age 18-75 years, any gender; 6. Healthy control group matched in age and gender with the DPNP group (+/-5 years); fasting blood glucose <6.1 mmol/L, HbA1c <5.7%; no history of chronic pain or neurological diseases.
Exclusion criteria
Exclusion criteria: 1. Nerve issues from other causes (like heavy drinking, vitamin B12 deficiency, etc.); 2. Serious liver or kidney problems (ALT/AST more than 3 times the normal limit, eGFR below 60 mL/min/1.73m^2); 3. Recently (within 3 months) taking bile acid supplements; 4. Women who are pregnant or breastfeeding; 5. Cancer or autoimmune diseases.
