18 Years to 100 Years

Inclusion criteria

Inclusion criteria: 1.Aged >=18 years; 2.Permanent Shenzhen residents (who have lived in Shenzhen for at least six months in the past one year); 3.Diagnosed as heart failure with III-IV NYHA according to National Heart Failure Guideline 2023; 4.Wiling to accept and comply with follow-up requirements and daily monitoring; 5.Signed informed consent.

Exclusion criteria

Exclusion criteria: 1.Had the major cardiovascular events one month prior to recruitment, such as myocardial infarction, heart surgery, stroke; 2.Had the plan or the possibility to receive heart transplantation or a ventricular assist device; 3.Pacemaker-dependent patients (pacemaker-dependent frequency >=40%); 4.Those who are unable to conduct the questionnaire survey due to serious illness, mental disorder, or difficulty in communication; 5.Dead during hospitalization at baseline.
