Interventions

experimental group:The intervention program consists of 8 sessions. On the day of discharge, patients participate in the first four sessions, which include health education to modify risk perception,
session 4, cooperate with the healthcare provider to complete a plan sheet. send an Exercise Log Form to record exercise process. session 5: making the coping plan
session6: self regulation skill
session 7and 8: motivational interview if needed.
control group:usual care

20 Years to No maximum

Inclusion criteria

Inclusion criteria: 1. are at least 20 years old; 2. with a confirmed diagnosis of CHD and received the PCI; 3. ability to speak and listen in Mandarin or the local language (Chaoshan language); 4. able to use the telephone/WeChat; 5. no severe comorbidities (tumor malignancies, severe liver and kidney disease, severe lung disease).

Exclusion criteria

Exclusion criteria: 1. have a life expectancy of <6 months; 2. an inability or unwillingness to follow up or give informed consent; 3 have been prescribed restricted mobility due to comorbidities such as aortic stenosis and dysrhythmias; 4. left ventricular ejection fraction less than 30%.
