Patients with stroke who met the diagnostic criteria for cerebrovascular diseases according to the Diagnostic Criteria for Major Types of Cerebrovascular Diseases in China 2019 of the Neurology Branch of the Chinese Medical Association, and who received their first definite diagnosis via cranial CT or MRI.

Inclusion criteria
Inclusion criteria: 1. Aged 18 to 60 years. 2. Patients with stroke who met the diagnostic criteria for cerebrovascular diseases specified in *Diagnostic Criteria for Major Types of Cerebrovascular Diseases in China (2019)* issued by the Neurology Branch of the Chinese Medical Association, with a primary confirmed diagnosis identified by cranial CT or MRI. 3. Patients in the recovery phase with stable clinical conditions. 4. Alert patients without verbal communication impairments, capable of sending and receiving messages via smartphones. 5. Participants who are willing to cooperate with the study, give informed consent, and sign the written informed consent form.
Exclusion criteria
Exclusion criteria: 1. Patients who are critically ill or in the end stage of a disease. 2. Patients who are completely dependent in daily living (i.e., completely unable to care for themselves). 3. Patients with a definite diagnosis of dementia or psychiatric disorders.
