Interventions
A group:Empagliflozin 10 mg orally once daily in addition to standard heart failure therapy for 6 months.
B Group:Standard guideline-directed medical therapy for heart failure, including diuretics, ACEI/ARB/ARNI, beta-blockers, and mineralocorticoid receptor antagonists as clinically indicated.

Inclusion criteria
Inclusion criteria: 1.The patient's clinical symptoms at presentation and relevant laboratory test results met the diagnostic criteria for heart failure according to the Chinese Guidelines for the Diagnosis and Treatment of Heart Failure (2022). 2.The patient was aged over 60 years. 3.The patient had a New York Heart Association (NYHA) functional classification of Class III or IV at enrollment. 4.The patient had complete clinical data and demonstrated good treatment compliance. 5.Echocardiographic examination showed a left ventricular ejection fraction (LVEF) < 50%.
Exclusion criteria
Exclusion criteria: 1.Patients with allergy to the drugs used in this study; 2.Patients complicated with contraindications such as cardiogenic shock and hypovolemic hypotension; 3.Patients with pericardial diseases; 4.Patients complicated with severe cerebral hemorrhage, cerebral infarction and other cerebrovascular diseases; 5.Patients with acute or chronic infectious diseases; 6.Serum creatinine > 221 µmol/L or estimated glomerular filtration rate (eGFR) < 45 mL/(min·1.73 m^2).
