Inclusion criteria

Inclusion criteria: 1.Age >= 18 years; 2.Confirmed diagnosis meeting the Sepsis-3 criteria: presence of suspected or confirmed infection, with an acute increase in the Sequential Organ Failure Assessment (SOFA) score by >= 2 points from baseline; 3. The patient or their legal representative signs a written informed consent form.

Exclusion criteria

Exclusion criteria: 1.Life expectancy of less than 6 months; 2.Presence of severe communication impairment / barriers; 3.Diagnosed with conditions such as stroke or severe traumatic brain injury resulting in a modified Rankin Scale (mRS) score of >= 4 at discharge. 4.Inability to complete the follow-up; 5. Refuse to participate
