Inclusion criteria
Inclusion criteria: Criteria for including participants in the intervention plan consultation: 1. Aged >= 65 years; 2. Diagnosed with diabetes by a hospital at or above the second-level level; 3. No communication barriers and able to express one’s own wishes. Criteria for including participants in the second part of the semi-structured interviews: 1. Aged >= 65 years; 2. Diagnosed with diabetic nephropathy by a hospital at or above the second level, with relatively stable condition and a history of treatment; 3. No communication barriers, able to express own wishes. Part 3: Feasibility study and inclusion criteria for randomized controlled trials: 1. Aged 65 years and above; 2. Diagnosed with diabetes by a hospital at or above the second level, with stable condition; 3. No severe visual or auditory impairments, able to understand and cooperate with the study; 4. Possess a smartphone and be able to use WeChat; 5. Voluntary participation and signing of the informed consent form.
Exclusion criteria
Exclusion criteria: Exclusion criteria for soliciting opinions on the intervention plan: 1. Patients with end-stage diabetic nephropathy; 2. Other organ damages, such as dysfunction of vital organs like the heart, liver, and lungs; 3. Patients in critical conditions and/or at the end-stage of their disease; 4. Patients who are unable to take care of themselves. Criteria for including semi-structured interviews in Part 2: 1. Patients with severe kidney disease caused by other factors, such as obstructive nephropathy or chronic nephritis syndrome, or those with other serious underlying diseases; 2. Patients with damage to other organs, such as dysfunction of vital organs like the heart, liver, and lungs; 3. Patients in critical condition and/or those at the end-stage of their disease; 4. Patients who are unable to take care of themselves. Part 3: Feasibility study and inclusion criteria for randomized controlled trials: 1. Patients diagnosed with diabetic nephropathy or end-stage chronic kidney disease (CKD); 2. Patients with comorbid severe chronic diseases (e.g., malignant tumors, heart failure class III or higher) or acute infectious diseases; 3. Patients exhibiting cognitive impairment (Simple Mental State Questionnaire [SPMSQ] score >= 3), long-term use of medications that may significantly impair cognitive function, history of psychiatric disorders, or impaired communication ability; 4. Participation in other similar intervention studies within the past 3 months; 5. Patients with unstable residence status or plans to relocate during the study period; 6. Patients with limited self-care capacity.
