Inclusion criteria
Inclusion criteria: 1. Voluntarily participate in this study and sign the informed consent form; 2. Aged >= 18 years, regardless of gender; 3. The average systolic blood pressure >= 140 mmHg and/or diastolic blood pressure >= 90 mmHg in two measurements at screening and baseline; 4. Those who have lived locally for 6 months and do not plan to relocate.
Exclusion criteria
Exclusion criteria: 1. Aged >= 80 years; 2. Secondary hypertension; 3. Patients with severe diseases such as tumors; 4. Pregnant women or those planning to become pregnant; 5. Refractory hypertension; 6. Patients with chronic kidney disease; 7. Patients with a life expectancy of < 6 months and who are unable to visit the township health center due to limited mobility; 8. Patients with cognitive impairment who are unable to complete the research; 9. Other patients who are evaluated by experts/doctors as unfit to participate in the study.
