Inclusion criteria
Inclusion criteria: Inclusion criteria for the case group: 1. Aged >= 18 years. 2. Severe stenosis or occlusion of the middle cerebral artery confirmed by MRA (MagneticResonance Angiography), CTA(Computed Tomography Angiography), or DSA(Digital Subtraction Angiography). 3. Able to cooperate in acquiring complete, high-quality MR imaging data. 4. No significant stenosis of the vertebrobasilar artery system. 5. Signed informed consent. 6. No significant functional disability prior to the current event, defined as a modified Rankin Scale (mRS) score of 0–1. Inclusion criteria for the control group: 1. Aged >= 18 years; 2. No significant stenosis of cerebral vessels confirmed by MRA, CTA, or DSA, and no significant intracranial abnormalities (e.g., infarction, space-occupying lesions, hemorrhage, or infectious lesions) on MRI; 3. Able to cooperate in acquiring complete and high-quality MR imaging data; 4. Provision of written informed consent.
Exclusion criteria
Exclusion criteria: Exclusion criteria for the case group: 1. Non-atherosclerotic intracranial artery stenosis or occlusion: stenosis or occlusion of the middle cerebral artery caused by embolism, tumor, trauma, vasculitis, congenital malformation, infection, or radiation injury. 2. Bilateral severe stenosis or occlusion of the middle cerebral artery. 3. Pre-existing lesions in the thalamus. 4. Contraindications to magnetic resonance imaging. 5. Life expectancy <= 1 year. 6. Prior diagnosis of neurocognitive disorders such as Alzheimer's disease, vascular dementia, dementia with Lewy bodies, or frontotemporal dementia. Exclusion criteria for the control group: 1. Contraindications to MRI examination; 2. Life expectancy <= 1 year; 3. Prior diagnosis of neurocognitive disorders, including Alzheimer's disease, vascular dementia, dementia with Lewy bodies, frontotemporal dementia, or other related conditions.
