60 Years to 90 Years

Inclusion criteria

Inclusion criteria: 1. Meets the diagnostic criteria for CHF; 2. Patients classified as NYHA functional class II–III; 3. Age >= 60 years; 4. Frailty Phenotype (FP) score > 0; 5. Provides informed consent to participate in this study.

Exclusion criteria

Exclusion criteria: 1. Presence of severe comorbid diseases or complications, such as malignant tumors, severe infections, etc.; 2. Hearing or vision impairments that lead to communication difficulties; 3. History of psychiatric disorders or presence of severe impairments in comprehension, expression, memory, or orientation; 4. Participation in other interventional studies at the same time.
