Inclusion criteria

Inclusion criteria: Those who are 65 years old or older and have one of the following problems; 1. There are >=2 chronic diseases; 2. There is a decline in daily activity ability; 3.>=5 kinds of medications; 4. Presence of geriatric syndrome; 5. There are social support issues (such as living alone, lack of social support, etc.); 6. Other circumstances that require assessment; 7. Agree to participate in this study;

Exclusion criteria

Exclusion criteria: 1. Terminal stage of disease, end-stage dementia, bedridden with disability, or patients with acute critical illnesses; 2.Refusal to participate in the study.
