Inclusion criteria

Inclusion criteria: 1. Age >= 18 years. 2. Scheduled for elective cardiac surgery (coronary artery bypass grafting [CABG], cardiac valve surgery, or combined CABG and valve surgery). 3. High perioperative respiratory risk (Assess Respiratory Risk in Surgical Patients in Catalonia [ARISCAT] score >= 45).

Exclusion criteria

Exclusion criteria: 1. Body mass index (BMI) > 40 kg/m². 2. Pre-existing neuromuscular disease. 3. Untreated tension pneumothorax. 4. Active pulmonary hemorrhage. 5. Left ventricular ejection fraction (LVEF) < 35%. 6. Refractory hypotension or arrhythmias. 7. Planned reoperation within 48 hours. 8. Severe underlying respiratory disease requiring long-term home oxygen therapy or noninvasive ventilation. 9. Severe dysfunction of other organs with expected short-term (7-day) mortality or palliative care needs. 10. Pregnancy. 11. Failure to provide informed consent.
