Inclusion criteria
Inclusion criteria: 1.Age >= 18 years old; 2.It meets the diagnostic criteria for chronic heart failure as stipulated in the "2018 Chinese Guidelines for Diagnosis and Treatment of Heart Failure", and has been diagnosed by a cardiologist as chronic heart failure with New York Heart Function Classification at grade II-IV. 3.Voluntarily participate in this research;
Exclusion criteria
Exclusion criteria: 1.Those with severe cognitive or behavioral functional impairments; 2.Including those with severe hearing or vision impairments who are unable to communicate; 3.Those with concurrent malignant tumors or other terminal diseases;
