Inclusion criteria
Inclusion criteria: 1. Age >= 60 years; 2. Diagnosis of at least two of the following conditions (ICD-10): primary hypertension, type 2 diabetes or impaired glucose regulation, dyslipidemia, coronary heart disease, heart failure, or stroke; 3. Alert and oriented, with basic communication skills; 4. Informed consent.
Exclusion criteria
Exclusion criteria: Suffering from severe mental disorders or having limited language communication and expression abilities.
