Inclusion criteria

Inclusion criteria: 1. Phase 1: (1) Discharged patients from 7 gastrointestinal surgery wards in 3 comprehensive tertiary hospitals in Jiangxi Province from January 1, 2026 to February 1, 2026; (2) Patients with a hospitalization duration >=24 hours. 2. Phase 2 (Nurses): (1) Registered Nurse; (2) Gastrointestinal surgery experience >=6 months; (3) Independent completion of NRS 2002 assessment for >=10 cases; (4) Informed consent to participate in this study, with signed informed consent form (due to nurse blinding, delayed disclosure was adopted to ensure authentic assessment habits in real-world settings). 3. Phase 2 (Patients): (1) Newly admitted or transferred patients in the Department of Gastrointestinal Surgery; (2) Patients aged >=18 years; (3) Conscious and capable of cooperating with the completion of information collection required for screening. 4. Phase 3: (1) Registered Nurse; (2) >=6 months of experience in gastrointestinal surgery; (3) Independently completed NRS 2002 assessment for >=10 cases; (4) Informed consent to participate in this study and signed the informed consent form.

Exclusion criteria

Exclusion criteria: 1. Phase 1: (1) Incomplete medical records, unable to obtain weight or height data. 2. Phase 2 (Nurses): (1) Advanced training nurses, and standardized training nurses. 3. Phase 2 (Patients): (1) Incomplete data, unable to obtain accurate weight or height measurements (e.g., amputation, severe edema, or inability to measure due to bedridden status). 4. Phase 3: (1) Advanced training nurses and standardized training nurses; (2) Absent from duty during the study period (2 weeks prior to interviews), such as on sick leave or attending advanced training programs.
