Inclusion criteria
Inclusion criteria: 1. Age >= 18 years old; 2. Diagnosed with PICS; 3. Aware of the condition; 4. Capable of independent reading and writing; 5. Residing in rural areas; 6. Have at least one smart phone and be proficient in operating it; 7. Willing to cooperate with the use of remote health management programs for self-management; 8. Voluntarily participate in this study.
Exclusion criteria
Exclusion criteria: 1. Unable to take care of oneself or uncooperative, or with mental disorders; 2. Unstable condition, in critical condition or a terminally ill patient.
