Inclusion criteria
Inclusion criteria: 1. Age >= 40 years, meeting the grading criteria for the high-risk population of stroke; 2. Able to understand the study content, capable of effective communication, and able to independently use a smartphone and WeChat; 3. Stable residence during the study period, able to comply with regular follow-up visits; 4. Voluntary participation in the study, agreement to receive the intervention methods of this study, willingness to comply with study arrangements, and signing of the informed consent form.
Exclusion criteria
Exclusion criteria: 1. Presence of unstable medical condition, such as unstable angina, myocardial infarction, heart failure, severe arrhythmia, significant hepatic or renal dysfunction, malignancy, or end-stage disease within the past 12 months; 2. Individuals with comorbid mental illness, cognitive behavioral abnormalities, communication difficulties, or limited activities of daily living; 3. Pregnancy status or have definite plan for pregnancy; 4. Unwillingness or inability to sign the informed consent form.
