Inclusion criteria

Inclusion criteria: 1. Age =65 years old; 2. =2 chronic diseases combined; 3. Received treatment with =5 different medications during hospitalization.

Exclusion criteria

Exclusion criteria: 1.Expected hospital stay < 48 hours or unable to complete follow-up; 2.atients in the terminal stage or those with critically severe conditions who are unable to assess their medication usage; 3.transferred to another hospital or whose data were incomplete during the study period;
