Inclusion criteria

Inclusion criteria: 1. Age >= 18 years; 2. Patients undergoing elective conventional open thoracotomy cardiac valve surgery under cardiopulmonary bypass; 3. Intermediate-to-high risk according to the preoperative ACEF score; 4. Written informed consent signed by the patient or their legal representative.

Exclusion criteria

Exclusion criteria: 1. Patients with CKD stage 5 or undergoing renal replacement therapy; 2. Pre-existing acute kidney injury; 3. Pregnant women; 4. History of nephrectomy; 5. History of kidney, liver, heart, or lung transplantation; 6. Patients with severe infection or sepsis; 7. Emergency surgery or time-limited surgery; 8. Expected requirement for circulatory arrest during operation; 9. Participation in other clinical trials that may interfere with the results of this study; 10. Preoperative hemodynamic instability requiring high-dose vasoactive agents (e.g., norepinephrine > 0.1 µg/kg/min) or intra-aortic balloon pump (IABP) support.
