Inclusion criteria
Inclusion criteria: 1. Age >= 18 years 2. Meets diagnostic criteria in Chinese Guidelines for Diagnosis and Treatment of Acute Ischemic Stroke; confirmed as first-ever acute ischemic stroke by brain MRI 3. Onset within 72 hours 4. Cognitive impairment present, Montreal Cognitive Assessment (MoCA) score < 26 5. Written informed consent signed by patient or legal representative
Exclusion criteria
Exclusion criteria: 1. Transient ischemic attack (TIA) 2. Stroke caused by other reasons such as cardioembolism or vascular malformations 3. Coexisting intracranial infection, malignant tumors, or severe hepatic/renal insufficiency (ALT/AST > 2x upper limit of normal, Scr > 133 µmol/L) 4. Allergy to piracetam or Ginkgo biloba leaves 5. Pregnant or lactating women 6. Severe psychiatric disorders or inability to cooperate with cognitive function assessment
