Inclusion criteria
Inclusion criteria: 1. Residents under the management of Nanshitou Street Community Health Service Center; 2. Age >= 35 years old; 3. Suffering from at least two chronic diseases (including but not limited to: hypertension, diabetes, chronic obstructive pulmonary disease, coronary heart disease, stroke, osteoporosis, etc.); 4. Having complete assessable data on treatable characteristics (based on the TTs chronic disease management system, including some data that can be supplemented later).
Exclusion criteria
Exclusion criteria: 1. Missing data for key variables > 20%; 2. Expected survival time < 6 months (such as advanced tumors).
