Interventions
Intervention group:1.4 weeks of 2-hour weekly sessions,
2.1 home visit,
3.5 online guidance and counseling sessions,
4.2 community activities and experience-sharing events.
Comparison group:1.Pharmacotherapy management
2.Lifestyle guidance
3. Regular follow-up (6 month and 12 month)

60 Years to 79 Years
Inclusion criteria
Inclusion criteria: .Aged 60-79 years,who had at least 3 of 8 risk factors, including 1.1 Hypertension(SBP=140mmHg and/or DBP=90mmHg), 1.2 Dyslipidemia (LDL>100mg/dl or >3.4mmol/L), 1.3 Diabetes Melitus (Fasting blood sugar > 126 mg/dl or >7.0mmol/L) 1.4 Smoking(Individuals who have smoked continuously or cumulatively for six months or longer) 1.5 Exercise 1.6 Obesity(BMI>28Kg/m2) 1.7 Atrial fibrillation(Past medical history/Irregular heartbeat), 1.8 Family history of stroke 2. Permanent residents ( =6 months living in this community) 3. Able to manage daily living activities independently as assessed by Activities of Daily Living scal (ADL:90-100 scores) 4. Can communicate, read and write in Chinese 5. No cognitive impairment as assessed by Mini-Mental State Examination (MMSE:27-30 scores) 6. Having no depression as assessed by Geriatric Depression Scale(GDS-15:0-4 scores)
Exclusion criteria
Exclusion criteria: 1. Those with terminal illnesses, such as cancer, kidney failure, heart failure etc. 2. Those who get admitted to the hospital. 3. Were not willing to participate in the program
