18 Years to 75 Years

Inclusion criteria

Inclusion criteria: 1. Patients admitted to our hospital's cardiology department with heart failure confirmed by a clinician; 2. Patients who meet the diagnostic criteria and classification of heart failure, with cardiac function at class II to V; 3. Patients aged 50 to 75 years; 4. Patients whose condition permits and who voluntarily participate in this study; 5. Patients who are able to proficiently use mobile phone apps after training.

Exclusion criteria

Exclusion criteria: 1. Not diagnosed with heart failure; 2. Patients with severe impairment of important organ function or other serious diseases (such as malignant tumors, etc.); 3. Patients with low intelligence or cognitive dysfunction who cannot express themselves correctly; 4. Patients who do not have the conditions to use mobile networks.
