Inclusion criteria

Inclusion criteria: 1.Be clinically confirmed with at least two chronic diseases; 2.Be over the age of 60; 3.Have the ability to read, write, or communicate effectively, and be able to complete questionnaires accurately alone or with researcher guidance; 4.Have obtained informed consent from both the patient and family members, and voluntarily participate in this study.

Exclusion criteria

Exclusion criteria: 1.Having impaired consciousness or a history of psychiatric disorders.
