Inclusion criteria
Inclusion criteria: 1. Patients aged >= 60; 2. Patients with an American Society of Anesthesiologists (ASA) physical status classification of II-III; 3. Patients to undergo lower extremity orthopedic surgery.
Exclusion criteria
Exclusion criteria: 1. Lower extremity fracture caused by high-energy trauma, or lower extremity fracture complicated with severe trauma to the craniocerebral, thoracic or abdominal regions (Definition of high-energy trauma: fall height >= 3 meters, motor vehicle collision, heavy object impact, etc.); 2. Moribund state on admission (expected to die within 24 hours) or severe organ failure (e.g., multiple organ dysfunction syndrome, MODS); 3. Preoperative cognitive impairment, defined as: based on previously validated norms and epidemiological evidence (with the influence of educational level taken into account), the score of Mini-Mental State Examination (MMSE) is lower than the education-adjusted threshold (illiteracy <= 17 points, primary school education <= 20 points, middle/technical secondary school education <= 22 points, senior high school and above education <= 23 points); 4. History of previous neurological diseases (e.g., Parkinson's disease, sequelae of stroke, epilepsy, multiple sclerosis, central nervous system infection, etc.); 5. Long-term use of psychotropic drugs, glucocorticoids/immunosuppressants before surgery (continuous use within the recent 3 months); 6. Severe visual, auditory or language communication impairment, unable to cooperate with the 3-Minute Confusion Assessment Method for delirium evaluation and relevant research procedures; 7. Inability to complete specimen collection or follow-up .
